Job Description:
Responsibilities include: Facilitating the development and implementation of the Home & Hospice Care of Rhode Island model of Hospice in nursing homes Providing Hospice in-service education to facility staff about Hospice Care in nursing homes Developing and maintaining excellent working relationships with facility Nursing Directors, Social Workers and MDS Coordinators

Requirements:
Bachelor of Science Degree in Nursing preferred or two years nursing home or hospice experience; or equivalent combination of education and experience.
